Practical training to help healthcare professionals support young people as they transition from children's to adult services.
As young people move from childhood to adulthood, they can experience gaps in our healthcare system.
Too old for children’s healthcare, not yet ready for adult systems. It's a vulnerable time and will inform their well-being and engagement with healthcare for the rest of their lives.
We’ve worked with South Yorkshire and Bassetlaw (SYB) Acute Federation in partnership with Sheffield Teaching Hospitals Foundation Trust and Sheffield Children's Foundation Trust to bridge that gap and empower staff to provide developmentally appropriate healthcare.
The training is suitable for anyone who works with young people - whether through direct care, supporting them during hospital visits, or helping them plan their appointments.
